Personal Health Records (PHRs) are electronic applications through which individuals can enter, modify, maintain and manage their health information in a secure environment at the convenience of their computer.

A PHR contains information about your medical health history, family health history, emergency contacts, physician information, insurance information, vision, dental, immunizations, hospitalizations, surgeries, allergies, infectious diseases, diagnostics/lab information, medical devices and physical/occupational therapies.

 

Why Should One Complete a PHR?

PHR provides a comprehensive approach to represent your health and can be customized to include your medical health history, family health history, prescription related information, and pathological tests you have undergone, surgeries/procedures and other health related information.

This report can be printed and carried along with you to your doctor or dentist, for a second opinion. We suggest carrying this with you during while traveling in case of any unforeseen medical emergency.


How long would it take to complete a PHR?

Approximately 10 minutes.

Just remember that you can always return to this record to update your information at any time in case you are not able to complete all the information in one shot.
